MITCHELL – Blaze Leeann Dawn Lively, infant daughter of Travis Ray and Amber Dawn (Livingston) Lively, was stillborn Tuesday, November 18, 2014, at St. Vincent Dunn Hospital in Bedford.

Surviving:          parents, Travis and Amber Lively, Mitchell
                       brothers, Jamison Lynn Ray Lively
                                      Blake Maxwell Lively and
                                      Jacob Scott Lively, all at home

Graveside service will be at 1:00 p.m. Thursday, November 20, at Mitchell Cemetery with Sister Becky Ryan officiating.
